- NPF-7 BLIND PERFORMANCE TEST INVESTIGATION RESULTS Pursuant to 1 O CFR 26, Appendix A, Subpart B, §2.8 (e)(4), on May 4, 1993, Virginia Electric and Power Company performed an investigation concerning blind performance testing results. Details of the apparent discrepancy and the ensuing investigation are included in Attachment 1. A letter from the HHS-certified laboratory _

ATTACHMENT 1 BLIND PERFORMANCE TEST INVESTIGATION RESULTS e
BLIND PERFORMANCE TEST INVESTIGATION RESULTS During a review of blind performance test results, a sample (Specimen No.
02874316400), which had screened as positive for methaqualone (~750 ng/ml) at our on site testing facility at Surry Power Station on January 27, 1993, was identified by Company Employee Health Services personnel to have been screened by our Department of Health and Human Services (HHS) certified laboratory with negative results (<750 ng/ml). On January 29, 1993, we directed our HHS laboratory to provide the quantitative value of the specimen (Specimen No. 02874316401 ). The follow-up gas chromatography/mass spectrometry (GC/MS) confirmatory test confirmed the presence of methaqualone at 607 ng/ml.
As a result, we requested our HHS laboratory to provide an explanation and documentation of their initial screening and subsequent confirmatory testing (GC/MS) performed on the specimen in question.
In the meantime, we independently verified that the subject blind sample provided by our supplier had indeed been spiked with methaqualone. A NIDA-certified laboratory performed screening and confirmatory testing on an aliquot from the batch from which the blind sample was produced. The aliquot screened positive for methaqualone using an 800 ng/ml standard and was then confirmed by GC/MS at 1020 ng/ml.
A copy of the HHS laboratory letter which describes the analysis process and results is enclosed in Attachment 2.

A request for retest at the limit of detection (LOD) for methaqualone was received on January 29, 1993.
The GC/MS ana1ysis confirmed the presence of methaqualone at 607 ng/mL.
This result was reported on February 5, 1993.
The test profile #725200 utilizes a mathaqualone acreening cut-off of 750 ng/mL.
A sample containing 607 ng/mL would screen "negative" and be reported as negative. All controls run with the initial test were within acceptable limits.
