Details of the apparent discrepancy and the ensuing investigation are included in Attachment 1. A letter from the HHS-certified laboratory concerning the follow-up investigation is included in Attachment 2.

e ATTACHMENT 1 BLIND PERFORMANCE TEST INVESTIGATION RESULTS

BLIND PERFORMANCE TEST INVESTIGATION RESULTS During a review of blind performance test results, a sample (Specimen No. 289-743-0317-0), which had screened as positive for amphetamine at our onsite testing facility at Surry Power Station on October 14, 1992, was identified by Company Employee Health Services personnel to have been screened by our Department of Health and Human Services (HHS) certified laboratory with negative results. We directed our HHS laboratory to provide the quantitative value of the specimen (Specimen No. 289-743-0317-1 ).

The follow-up gas chromatography/mass spectrometry (GC/MS) confirmatory test indicated a positive result for amphetamine.

We also requested our HHS laboratory to provide an explanation and documentation of the initial screening and subsequent confirmatory testing (GC/MS) performed on the specimen in question. In the meantime, we independently verified that the subject blind sample provided by our supplier had indeed been spiked with amphetamine.

Furthermore, we processed a special panel of amphetamine-spiked blind proficiency specimens and received correct results for each sample from both our onsite facility and HHS laboratory.

A copy of the HHS laboratory letter with a description of analysis attached is included in Attachment 2.

Specimen No. 289-743-0317-0 Description of Analysis:

The specimen referenced above was received at the laboratory on October 14, 1992.

The specimen was analyzed and reported as "negative" on October 15, 1992.

A review of the initial test data for amphetamines revealed an absorbance of 7 units less than the cutoff.

A sample must be equal to or greater than the cutoff to conduct confirmation testing.

Therefore, the confirmation test (GC/MS) was not performed.

The screening calibrator is validated prior to being put into use, and sequentially with each batch of specimens through the use cf positive and negative controls.

There was no indication that the calibration of cutoff was improperly determined.

The cutoff concentrations for the initial test are set to reflect 1000 ng/mL amphetamines.

The immunoassay product which is approved by the FDA utilizes a calibrator which contains 1000 ng/mL D-methamphetamine.

The immunoassay kit shows different responses to different compounds which comprise the "amphetamine" class.

For

example, D,L-amphetamine at 1000 ng/mL has a response similar to D-amphetamine at approximately 300-400 ng/mL.

1000 ng/mL of D-methamphetamine has a response similar to 1000 ng/mL of D,L-amphetamine. The isomers, L-amphetamine and L-methamphetamine at less than 2000 ng/mL and 7000 ng/mL, respectively, will yield a negative response.

Thus, the interpretation of a direct correlation between the concentration "spiked" into a QC sample and the immunoassay response depends on the exact composition and relative amounts of the isomer(s) present in the calibration standard and the "unknown" (QC) sample.
