REPLY TO A NOTICE OF VIOLATION
-NRC INSPECTION CONDUCTED APRIL 6
- APRIL 10, 1992 SURRY POWER STATION UNITS 1 AND 2 INSPECTION REPORT NOS. 50-280/92-09 AND 50-281/92-09 NRC COMMENT:
During an NRC inspection conducted on March 23-27 and April 6-10, 1992, ~
violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 1 O CFR Part 2, Appendix C (1990), the violation is listed below:
Surry Technical Specifications, Section 4.4, Item 2.2, and associated Table 4.2-1, as implemented by site Augmented *inspection Manual, requires that the Recirculation Spray Rings be visually and liquid penetrant inspected. The liquid penetrant inspection is to include six patches (each nine inches square) evenly distributed around the ring. The visual inspection is to cover the full length of pipe.
1 O CFR 50, Appendix B, Criterion V, requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures........ and shall be accomplished in accordance with these instructions, procedures.........
Paragraph 1.5.3 of licensee procedure NDE-PT-201, the applicable procedure for liquid penetrant inspection, requires that the surface to be examined and adjacent areas within at least one inch shall be dry, and free of all dirt, grease,
........ and other extraneous matter that could obscure surface openings or otherwise interfere with the examination.
Contrary to the above_, on March 24, 1992, activities affecting quality were not being accomplished in accordance with instructions and procedures in that, for one Recirculation Spray Rin*g (ISi Items AE-063, 064, 065, 068, 069 and 070),
three of the nine inch patches were liquid penetrant inspected without removing blotches of paint and other foreign materials (marking materials). In addition
_ only a nine inch band around the header at each liquid penetrant patch, in lieu of the full length of pipe, was being visually inspected.
This is a Severity Level IV Violation (Supplement I).

. INSPECTION REPORT NOS. 50-280/92-09 AND 50-281/92-09 Reason for the Violation or. if Contested. the Basis for Disputing the Violation The violation was due to personnel error in that a contract non-destructive examination
. (NOE) inspector failed to fully comply with the requirements of the liquid penetrant and visual examination procedures while performing those examinations on the Recirculation Spray Ring piping.
Corrective Steps Which Have Been Taken and the Results Achieved
. On March 25, 1992, contract NOE inspectors were assembled and informed of the failure to fully comply with liquid penetrant and visual inspection requirements which had occurred _during the previous day.
The inspectors were instructed on the importance of and requirement for procedural compliance. _ Surry Power Station surface examination procedures were reviewed with specific *emphasis on initial conditions and acceptance criteria.
The specific inspector involved in this violation was directed to return to the contractor's home office for additional training and retesting on the requirements of liquid perietrant examihations. The inspector' successfully completed fourteen hours of.
training at the contractor's home office and passed a re-examination administered by Surry Power Station personnel on March 27, 1992.
Each examination which had been performed on the Recirculation Spray Ring piping by contract NOE personnel was reperformed using proper procedures with no discrepancies noted.
In addition, a number of weld examinations previously performed during the outage by this inspector were randomly selected and re-examined by station personnel with no discrepancies noted.
Corrective Steps Which Will be Taken to Avoid Further Violations Review and re-performance of the NOE examinations performed by other contract NOE personnel confirm that this violation was an isolated event. Therefore, the corrective steps which have been taken and are described above* should prevent further violations.
The Date When Full Compliance Will be Achieved Full compliance was achieved on April 18, 1992 with the successful reperformance of the examinations on the.Recirculation Spray Ring piping.
