SUMMARY
OF MANAGEMENT SANCTIONS IMPOSED 1/1/91 THROUGH 6/30/91 VIRGINIA ELECTRIC AND POWER COMPANY EMPLOYEES RANDOM TESTS Confirmed Positive Tests for Illegal Drug(s) 2 One positive test occurred prior to a change in Company policy.
The employee's unescorted access authorization was revoked and permanently denied, and employment was terminated.
The second positive test occurred after the policy change, which resulted in the employee being placed on 30 calendar day Disciplinary Suspension (without pay). The employee was also referred to the Employee Assistance Program for rehabilitation, and is now subject to a follow-up testing program for the next three years.

SUMMARY
OF MANAGEMENT ACTIONS COMPLETED TO IMPROVE THE FFD PROGRAM 1/1/91 THROUGH 6/30/91 Although no weaknesses requiring corrective action were discovered during this period, the following activities were performed as enhancements to the program:
A Quality Assurance audit of the Fitness for Duty Program identified strengths in the program management, professionalism, program involvement and support, and the certification of the Virginia Power Employee Assistance Program personnel by the EAP Association.
Inquiries to law enforcement officials regarding potential forms of substance abuse have resulted in no changes to the substances for which testing is performed by the FFD program.
e EVENTS REPORTED TO THE NRC BY VIRGINIA ELECTRIC AND POWER COMPANY 1/1/91 THROUGH 6/30/91 Two blind proficiency samples that tested negative onsite subsequently tested positive for phencyclidine at the DHHS laboratory.
The incident was investigated and corrective actions were taken. A report was submitted to the NRG via company letter (Serial No. 91-165A) dated May 1, 1991.
A sample that screened negative onsite w~s sent to the DHHS laboratory as part of the blind performance testing of at least ten percent of samples in the program. The sample tested positive for phenobarbital at the DHHS laboratory. After investigation, it was determined that this result was not due to an error, but rather the inherent lack of sensitivity of the assays to phenobarbital. This sample falls into the category of the expected one percent failure rate associated with the low sensitivity of the screening assays to phenobarbital. This was reported to the NRC in our letter (Serial No.91-261) dated May 29, 1991.
Two blind proficiency samples that tested negative onsite later tested positive for methaqualone at the DHHS laboratory. The incident was investigated and corrective actions were taken. This was reported to the NRC in our letter (Serial No.91-277) dated June 5, 1991.
