License Nos.91-277 50-280 50-281 50-338 50-339 DPR-32 DPR-37 NPF-4 NPF-7 Pursuant to 1 O CFR 26, Appendix A, Subpart B, §2.8 (e), Virginia Electric and Power Company has conducted an investigation concerning unsatisfactory blind performance testing results. Following discovery of two false positive samples by our Employee Health Services Department personnel, the Department of Health and Human Services (HHS) certified laboratory determined that an incorrect cut-off level had been applied for methaqualone. This discrepancy resulted in deviation from the cut-off levels for initial screening and confirmatory gas chromatography/mass spectrometry (GC/MS) required by the Company's Fitness for Duty (FFD) Program.
These results were initially reported to Mr. E.W. McPeek*of the NRG NRR Safeguards Branch staff in a telephone conversation on May 8, 1991. Attachment 1 contains a summary of our investigation.

e INVESTIGATION RESULTS On April 25, 1991, Company Employee Health Services personnel, while reviewing specimen test results reported by our Department of Health and Human Services (HHS) certified laboratory, determined that a potential administrative false positive error had occurred on blind performance test specimens. The incident was promptly reported to the NRC via telephone in accordance with 1 O CFR 26, Appendix A,
§2.8(e)(5).
These results involved blind performance testing specimens for methaqualone which were processed at Surry and North Anna Power Stations. The samples had negative screening results (<750 ng/ml) when tested at the onsite testing facility. Subsequently, the samples were reported by the HHS laboratory as confirmed positives for methaqualone at approximately 400 ng/ml.
The false positive report for methaqualone was due to the HHS laboratory use of cut-off criteria other than the 750 ng/ml cut-off level stipulated by our FFD Program.
The HHS laboratory applied an initial screening cut-off level of 300 ng/ml and a confirmation cut-off level of 200 ng/ml.
It should be noted that methaqualone is not on the panel of drugs to be tested for according to 1 O CFR 26, Appendix A, Subpart B, §2.1 (a).
Furthermore, use of methaqualone has never been detected in actual specimens by our FFD Program despite the fact that the HHS laboratory had always used the lower cut-off levels.
Methaqualone has only been detected in blind performance test specimens.
This discrepancy was corrected on May 2, 1991, when the screening and confirmation cut-off levels were adjusted to the proper program value of 750 ng/ml. No further actions are required with respect to administration of cut-off levels by the HHS laboratory.
