REVISED NRC FORM 398
Dear Mr. O'Hanlon:
May 28, 1998 Enclosed is a copy of the revised NRC Form 398 (10-1997), Personal Qualification Statement -
- Licensee. Effective immediately, all applications for new licenses or license renewals are to be submitted on the revised form. The form has been changed to reflect current operator licensing policy.
NRC Form 398 - Item #13, A minimum of five significant control manipulations must be performed which affect reactivity or power level under item 13.3.c. In accordance with 10 CFR 55.31 (b), the five significant control manipulations must be listed in item 17 on NRC Form 398.
