VIRGINIA ELECTRIC AND POWER COMPANY SURRY POWER STATION UNITS 1 AND 2 RADIOACTIVE WASTE SHIPMENT 30-DAY REPORT Pursuant to 10CFR71.95(c), this 30-day report is being submitted for two instances in which the conditions of approval in the certificate of compliance were not observed in making a shipment. .On July 23, 1999, we became aware that the Quality Assurance (QA) Program Approval for Radioactive Material Packages No. 0194, Revision No. 6 was not renewed in accordance with 10CFR71.38 and had expired on May 31, 1999. The reinstatement of the QA Program Approval was issued by a July 28, 1999 letter from the NRC. Between July 23, 1999 and July 28, 1999, all 10CFR71 shipments were suspended. In the period between May 31, 1999 and July 28, 1999, two Type B shipments using a Model No. CNS 8-1208 package were made. The Certificate of Compliance (COC) for the Model No. CNS 8-1208 package includes a condition that states the package is approved for use under the provisions of 10CFR71.12, and 10CFR71.12(b) requires a licensee to have a quality assurance program, satisfying subpart H of 10CFR71, approved by the Commission. The condition being reported in this letter is administrative in that our packaging practices, methods, and procedures had not changed, although the QA Program Approval had expired.

The cause of the approval expiration has been determined to be that this renewal requirement was not being tracked. A contributing factor has been determined to be that, following a personnel reassignment, this responsibility had not been properly turned over and reassigned.

The need for renewal of the current QA Program Approval, which expires on July 31, 2004, has been identified and is being tracked.
