Chc~ical Injury Inquiry was made into allegations that an employee was in danger of losing a finger as a result of radiation burns received while clean~ng up a plutonium nltrate leak. The licensee provided information that the naned employee had never worked in the plutonfo1:1 plant, He had, ho~aver, inc.urrcd a chemical injury to his left thumb on October 2, 1973. The employee had been instructed to clean the shut down pellet presses in the uranium plant, *water is norl'ilillly used for this, but the er.iployee obtained a metal cleaner used by rr.. 1 in te112nce personnel for other purposes. According to the label on the drum, the cle~ncr contains acid fluorides. Rubb~r ~lov~o ~ere ~orn during the cleanup and the employee washed *his h.'.l.nds 'before leaving the plant. Unusual Occurrences
- a.
At 1:30 p.~., Febru~ry 13, 1974, an employee eng~ged in changing out a pucp in Box SA., in Room 128, surveyed hls gloves after Temoving the:n from the glove box gloves and four!d thera to be contarainated, He put on his respirator.and health physics was notified. A hole found in one*of the glove box gloves is believed to be the cause of the contamination, Preliminary counts of the air samples in Room 126 show1::d an average concentration of 3 x 10-11 microcuries per i:lilliliter. These will be recounted after further decay of radon. The employee had contamination up to 100,000 disintegrations per minute on his ears. Nasal smears were 200 and 390 disintegrations per minute. The licensee believes that the contaoination was caused by the e!!!ployee's conta1:1inated gloves. The e~ployee was restricted from further exposure and pl~ced on a 5 day fecal and urine sa~pling progra.~. Results of these sam?les were not yet available. An in vil10 count was made on February 14, but results ~ere not yet available. The occurrence is still being evaluated by the licensee.

By telephone o~ January 7, 1974, the licensee informed Region III tha~ on January 5, an e:i,ployee working at Glove Bo~: 14 jabbed a piece of screen wire through the glove into his left forefinger. Su,:vey with a wound counter showed about 2.5 nanocuries of plutonium in the wound. On January 7, on the advjce of Kerr-McGee's medical consult~nt. a section of the finger about one-eighth inch deep by three-eighths of an inch long 'l.*as e:,cised to remove the plutoniu~. Resun,.,e'l of the wound sho~~d no detectable p1utoniu.:i. The ecployee is on a norcal urine sanpling progra~. Results are not yet available. - facility found eeverd 1011-enriched unnium d:lo:dcle pellet& and pellet fragtllt!nts oD the ground near the Safeguard* office building. *The finding pTompted a thorou&h 1e1rcb of-about 7500 equare feet of area which resulted in the. retrieval of pellet material equivalent to about 2S pellets~all within the perimeter fence of the Cimarron facility, but the &ajor portion outside the protected area fence that aQrrounda the uranium =anufacturing building.
Licensee management vas unable to determine the reason for the presence of ur1111lU111 pellet* in the unusual locations, but concluded that the cause could not have,een *ccidental. R0:111 va1 notified of the incident

After all the pelJet 1Daterial.h1d been collected by the Contracts Supervisor it was examined ~nd ve~ghed. The tota~ weight vas equivalent to about 25 pelleu. l'roai identifying letters stamped on the ends of the pellets, it vas determined that the dlliterial vi& from a current production lot and a 1~~ v~ose.;roduttJon had recently been completed on Decll!lllber 10, 1974. The Security Supervilft>r *stated th!t all cployeea are toutinely being ~uney~d by Real~h Ph7aica technicians on exitina t'lte building at ahift change to prevent the po11lbility of uranium facilitJ e111ployees lea~g the building tn a cont,1111inated condition. Thi1 survey had been in effect for about tvo veelr.s prior to the diacovery of the pellet& outside the buildins. The Contracts Supervieor aaid that eoon 8fter the finding of the pellets, auperv!1ion of the uranium facility *had conducted a search for *ny means ti)' which pelletl could have been ahot or propelled to the locations where they vere found. The.,earch d1~cloeed only a 8111511 device in the form of a handgun ciade or pipe fittings and a apring and held together v1th plastic tape. An effort to u1e the device disclosed that it would propel a pellet lea, than three feet. The Superintendent of the uranium facility explained the pre&ence of this object. Be 1aid that although be bad not known of f.tleJCistence, 11everal supervisors and operators bed bow ft exbted. Re aaid that it vas comon knowledge that the device bad been *de by an inventive employee'sbout two years ago. then had beea abandoned and left in a maintenance cabinet vhere it had been found dudn& the.eaTCh. The Cimarron Facility Manager told the investigator early on December 19 that the FBI office in Oklahoma City had been iaformed of the pellet incident in addition to RO:IlI. He also said that news media had not been informed. However, evening newspapeTS and television news broadcasts in Oklahom.s City on December 19 ref,rred to the pellets as vell ae to other incidents that had Tecently occurred at the Cimarron Facility. /
