SURRY POWER STATION RESPONSE TO NOTICE OF VIOLATION APPENDED TO INSPECTION REPORT 50-280/81 50-281/81-05 Attachment Page 1 NRC COMMENT 10 CFR. 50, Appendix B, Criterion IX implemented by VEPCO Topical Report VEP-1-3A, Section* 17, Paragraph 17.2.9 requires in part that measures shall be established to assure that special processes,- including nondestructive testing are controlled and accomplished using qualified procedures in ac-cordance with applicable codes, standards, *specifications, criteria, and other special requirements-.
The applicable code for radiography specifies that the film density of the area of interest shall not be less than 15% nor more than 30% of the film density of the penetrameter.
Contrary to the above, several radiographs of two safety related piping welds had film densities in. the area* of interest that were more than 30 percent of the film density of the. penetrameter.
This is a. Severity*Level ~- Violation (Supplement II. E.).

The* film reader in this case* had recorded the density properly but failed to recognize that it was out of tolerance..
CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED; Welds W-17 and W have* been re-radiographed and the results were accep-table. All radiographs taken prior to this inspection have been reviewed by a Level III examiner.
No discrepancies were found, The film reader has been re-instructed as to the code requirements for film density.
CORRECTIVE STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS; All film will be reviewed by a. Level III examiner in addition to the film reader.
All film readers have received training upgrade. *
