Corrective steps to avo:i.d further deficiencies:
A detailed review of station valve line-ups is being conducted and additions or corrections to upgrade the line-ups are being made as necessary.
Additionally a MOP (Maintenance Operating Procedure) review is underway to insure valves addressed in removing systems or components from service are also addressed in returning them to service.
Generic Maintenance Operating Procedures will also be upgraded to address this potential problem.
