..:..i.o Under the provisions of the operating license Technical Specifications and/or 10CFR21, NRC Region II was notified on May 25, 1979, that a potential significant deficiency or substantial safety hazard existed at Surry Power Station, Unit No. 2.

In accordance with the reporting requirements of the above regulations, the following information is submitted to supplement our earlier letter dated May 31, 1979, Serial No. 423.

Facility, activity, and/or component affected:

The report is from Surry Power Station, Unit No. 2 as part of the Steam Generator Replacement Project.

The affected components are the lower shells of Steam Generator Nos. 2971, 2972 and 2973.

Name of firm supplying the component, activity, or service:

The components are supplied by Westinghouse Electric Corporation, Tampa, Florida.

d.

Description of defect, deficiency, or failure to comply:

The helicoils with discrepancies were:

Steam Generator No. 2971; 1, 2, 3, 4 and 5 Steam Generator No. 2973; 1, 4, 5, 6, 7, 9, 10, 12, 13, 16, 18 19, 21, 23 and 24 Steam Generator No. 2972; 1, 2, 4, 15, 16 and 19

Some discrepancies were found such as the holes being oversized and a widening near the bottom.

Bolt hole No. 4 on Pad No. 19D of Steam Generator No. 2973 was discovered to have approximately one half to three quarters of an inch of thread missing.

Similar components, activities, or services:

This potential deficiency could be present in the Westinghouse Series 51 Steam Generators, which have been or will be shipped to Virginia Electric and Power Company. (Generator No. 2981 has been shipped, and Generator Nos. 2982 and 2983 are on order).

g.

Corrective Action which has been, is being, or will be taken; the individual responsible and the length of time to complete the action:

1.

All helicoils of Steam Generator Nos. 2971, 1972 and 2973 feet were removed and new helicoils installed.

2.

The defective bolt holes were corrected using larger helicoils.

3.

Bolt hole No. 4 on Pad No. 19D of Steam Generator No. 2973 was fitted using a special long bolt (9 1/4") to ensure the required thread engagement.

NOTE:

An inspection of helicoils on the new steam generators for Unit 01 will be initiated to ensure that the thread engagement meets the requirement.
