'l'he report is from Surry Power Station, Unit No. 2 as part of the Steam Generator Replacement Project. 'l'he affected component is the lower shell of Steam Generator No. 2973.

Description of defect, deficiency, or failure to comply:
Bolt hole No. 4 on Pad No. 19D of Steam Generator No. 2973 was discovered to have approximately one half to three quarters of an inch of thread missing.
The helicoil was inserted in the bolt hole and the missing threads were not detected during receipt inspection.
Some problems were encountered 1n the fitting and the helicoil was removed and the problem detected.
'Ibis pro-blem could have resulted in less than required thread engagement.

Similar components, activities, or services:
'Ibis potential deficiency could be present in the Westinghouse Series 51 Steam Generators, which have been or will be shipped to Vepco (Generator
- Nos. 2971, 2972, 2973, and 2961 have been shipped).

Corrective action which has been, or is being, or will be taken, the individual responsible and the length of time to complete the action.
All belicoils of the steam generator feet were removed and new helicoils installed. 'l'he affected bolt hole will be fitted using a special long bolt (9 1/4") to ensure the required thread engagement.
An inspection of helicoils on the two remaining steam generators is underway to ensure that the thread engagement requirement can be met.
Investigation by Vepco and Westinghouse personnel is continuing.
Any additional information such as identification of additional corrective action will be submitted as a supplement to this report.
