PUBLIC SERVICE ELECTRIC AND GAS COMPANY SALEM GENERATING STATION CASE NO. 96-01-07-0049-18 SUPPLEMENTAL DISCHARGE CONFIRMATION REPORT In accordance.with the Discharge Confirmation Report submitted on February 6, 1996 (LR-E96019), Public Service Electric and Gas Company (PSE&G) is submitting this supplemental report on corrective measures to be*taken *to reduce the potential of future occurrences.

As discussed in the February 6 report, PSE&G inadvertently discharged approximately 115 gallons of sodium hypochlorite to the Delaware River. The cause of the incident was attributed to human error involving inattention to detail by Operations personnel. Contributing causes included inadequate maintenance of the sodium hypochlorite pump recirculation system and inadequate design of the chlorine building sump. The following corrective measures are being implemented:

1) This event and lessons learned shall be communicated to -

Operations personnel and shall focus on the proper use of PSE&G's self-checking principle (STAR ~top - ~hink - Action

- Review) and tagging procedures. This corrective measure shall be completed by May 15, 1996.

2) Several Action Requests have been initiated to address repairs to the sodium hypochlorite system. All corrective maintenance actions necessary to prevent recurrence .shall be completed prior to returning the system to service.

As discussed above, a single grab sample was obtained at the 13B intake bay at approximately 2:00 a.m. on January 7.

The sample was analyzed for Total Residual Chlorine via amperometric titration (EPA Method 330.1) at the Salem Generating Station (NJ Lab No. 17327).

FINANCIAL RESPONSIBILITY:

PSE&G satisfies the requirements for self-insurance established

by NJAC 7:1E-4.5(g) and has included appropriate documentation of this in its Discharge Cleanup and Removal (OCR) Plan as required by NJAC 7:1E-4.4(a)9.
