APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Units 1 and 2 Docket Nos: 50-272 50-311 License Nos: DPR-70 DPR-75 During an NRC inspection conducted on January 16 - February 19, 1994, a violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1992), the violation is listed below:
Salem Unit 2 Technical Specification 3.0.4 requires, in part, that entry into an Operational Mode not be made when: (1) the conditions of a Limiting Condition for Operation are not met, and (2) the associated Action Statement requires a unit shutdown if those conditions are not met within a specified time interval. Salem Unit 2 Technical Specification 3.7.1.2 requires that three steam generator auxiliary feedwater pumps be operable when the unit is in Modes 1, 2 and 3. Technical Specification 3.7.1.2, Action a., requires that with one auxiliary feedwater pump inoperable, the licensee shall restore the required pump to operaQle status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or place the plant in at least hot shutdown within the following 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.
Contrary to the above, during start-up operations on May 21, 1993, Salem operators placed Unit 2 into Operational Mode 3, which requires operability of three auxiliary feedwater pumps, though one of the auxiliary feedwater pumps was inoperable and subject to the constraints of the Technical specification 3. 7.1.2.
This is a Severity Level IV violation (Supplement I).
