Dear Ms. LaMarche:

I am responding to your February 19, 1992, request for information concerning an event that occurred at the Salem Nuclear Generating Station, Unit 2, on November 9, 1991.

The event was a turbine overspeed event that created excessive vibration in the main turbine resulting in, among other things, severe damage to the electrical generator. The damage resulted in lubricating lines being broken and hydrogen being released (the electrical generator is cooled by hydrogen).

The hydrogen ignited, which in turn ignited the oil. A combination of installed fire protection systems and the site fire brigade suppressed the fire in about 20 minutes.

Enclosed is a copy of the Region I Inspection Report that was issued by the Nuclear Regulatory Commission on January 7, 1992.

This report contains the details concerning the event and should answer your questions.
