Dear Sir:

SALEM GENERATING STATION LICENSE NO. DPR-75 DOCKET NO. 50-311 UNIT NO. 2 January 9, 1992 TECHNICAL SPECIFICATION SURVEILLANCE NOTIFICATION REQUIREMENT 4.4.6.5.a On December 30, 1992, Steam Generator {S/G) tube plugging was completed.

Attached is a list of the plugged tube locations completed this outage.

In all, eighteen {18) tubes were plugged of which:

two (2) tubes were plugged due to imperfection depths greater than or equal to 40%; six (6) tubes were plugged due to imperfection depths between 30 and 40%; eight (8} tubes were plugged by installation of a dampener followed by inconel 690 plug on the Hot Leg and a sentinel plug on the Cold Leg; and two (2}

tubes had their inconel 690 cold Leg plug replaced with a sentinel plug.

Tube dampening and plug replacement was completed as per Westinghouse recommendations, discussed in PSE-91-197 dated October 30, 1991, due to the susceptability to flow induced fatigue phenomenon in the U-Bend region of the subject tubes.

This notification is being made, within fifteen (15) days after tube plugging, as required by Technical Specification Surveillance 4.4.6.5.a.
