ATTACHMENT 1 NLR-N91134 Responses to NRC Bulletin 89-01, Supplement 2:
Item 1 Table 2 of Reference 4 was reviewed for Salem Unit Nos. 1 arid 2.
The results of this review were as follows:
- Salem Unit 1 -
The table does not reflect.the current information from the Spring, 1991 refueling outage.
During this outage, all Inconel 600 mechanical plugs in the hot leg of all four steam generators were drilled out and.replaced with Inconel 690 plugs from heat NX-722iHK.
The plugs in.
the cold leg remain as listed in the report.
The following discrepancy should also be noted.
The original number of plugs listed in 14 steam generator hot leg Should have been 121, instead of 120.
There was one additional Inconel 600 plug that was installed from either heat NX-6135 or NX-6323 in *4-89.
The plug operating temperatures listed in the table are correct as listed.
Salem Unit 2 -
The number of plugs installed from some of the heats listed in the table contain minor inaccuracies.
However, the general status of.the current repairs and replacements which have been made.to these heats to date are correct.
The plug operating temperatures listed in the table are also correct as stated. is a copy of the Salem Unit 2 table with the marked up corrections for the inaccuracies identified above.
Item 2a All the mechanic.al plugs in both Salem units whose estimated lifetimes would not extend to the next refueling outage have been either repaired by the installation of the Westinghouse plug-in-plug (PIP) technique, which is the repair technique recommended by Westinghouse in WCAP-12244, or have.been drilled out and replaced with Inconel 690.
Item 2b There are no corrections which are required to be made to the column entitled "Reference CALC Dates" based on our response to Action 1 above.
J, NLR-N91134 Item 2c Salem Unit 2 is our only plant in which.plugs fabricated.
from heat NX-5222 were installed.
Below are ~he results of the actions taken or to be taken for these plugs.
Steam Generator # 21 Hot Leg -
77 plugs installed, all repaired by the installation of a Westinghouse.PIP.
Cold Leg -
None installed.
Steam Generator #22 Hot Leg -
61 plugs installed, all repaired by the installation of a Westinghouse PIP.
Cold Leg -
31 plugs install~d.
No action to be taken at this time due to lower operating temperatures and calculated remaining full power days which currently exceed the expected lifetime of the steam generator.
- Steam Generator #23 Hot Leg -
None installed.
Cold Leg -
82 or 83 plugs installed.
No action to be taken at this time due to lower operating temperatures
\\
and calculated remaining full power days which currently exceed the expected lifetime of the steam generator.
Steam Generator #24 Item 2d Hot Leg_- 77 plugs installed, all repaired by the installation of a Westinghouse PIP.
Cold Leg -
31 plugs installed.
No action to be taken at this time due to lower operating temperatures and calculated remaining full power days which currently exceed the expected lifetime of the steam generator.
All plug installations since 1990 have been Inconel 690.
Future plugging will also be performed with Inconel 690 or other approved material.
Inconel 600 plugs are no longer used. in the steam generators.
- NLR-N9i134 Item 2e No action is required for this item.
Item 2f ALARA considerations are always carefully considered and weighed before performing any work activity in the primary side of the steam generators.
Currently most of the primary side work is being performed using robotics.
