SALEM GENERATING STATION LICENSE NO. DPR-75 DOCKET NO. 50-311 UNIT NO. 2 SPECIAL REPORT 90-8 June 13, 1990 This Special Report addresses the circumstances surrounding the inoperability of the automatic actuation capability of the Diesel Generator Area C02 Fire Protection System for greater than 14 days.

This report is required per Technical Specification Action Statement 3.7.10.3a pursuant to the requirements of Technical Specification 6.9.2.

This report is required thirty (30) days after the system has been inoperable for greater than fourteen (14) days.

Diesel Generator Area C02 Fire Protection System Inoperable For Greater Than Fourteen Days Event Date(s):

6/13/90 This report was initiated by Incident Report No.90-320.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 6 (Refueling); Day 48 of the 5th Refueling Outage DESCRIPTION OF OCCURRENCE:

This special report describes the inoperability of the automatic actuation capability of the Diesel Generator (D/G) Area C02 Fire Protection System for greater than fourteen (14) days.

This report is submitted for informational purposes in accordance with Technical Specification Action Statement 3.7.10.3a which states:

"With one or more of the above required C02 systems inoperable, within one hour establish a continuous fire watch with backup fire suppression equipment for those areas in which redundant systems or components could be damaged; for other areas, establish an hourly fire watch patrol.

Restore the system to OPERABLE status within 14 days or, in lieu of any other report required by Specification 6.9.1, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next 30 days outlining the action taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status.

On May 3, 1990 at 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br />, the Diesel Generator (D/G) Area C02 Fire Protection System was made inoperable in support of D/G work.

As of May 17, 1990, the D/G Area C02 Fire Protection System automatic actuation feature was still inoperable.

APPARENT CAUSE OF OCCURRENCE:

The disabling of the C02 fire protection system was part of a planned work effort.

"ANALYSIS OF OCCURRENCE:

The DIG C02 Fire Protection System is designed to mitigate the consequences of a fire in the D/G areas automatically.

Disabling the automatic actuation feature was a planned evolution.

This was done in support-of planned D/G maintenance in order to protect the workers in the area against inadvertent actuation.

As required by Technical Specification Action Statement 3.7.10.3a, a fire watch was in place.

during the period when the automatic actuation feature was inoperable.

Therefore, this event did not affect the health or safety of the public; however, this event is reportable to the Nuclear Regulatory Commission in accordance with the requirements of Technical Specification Action Statement 3.7.10.3a pursuant to Technical Specification 6.9.2.

CORRECTIVE ACTION:

.The C02 System was returned to operable status on May 21, 1990 at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> upon completion of D/G maintenance activities.
