On February 9, 1989, Public Service Electric and Gas Company (PSE&G) submitted a license change request (LCR) to amend Appendix A of Facility Operating License No. DPR-70.
This license change would have provided for an alternate steam generator tube sampling method to that described in the Technical Specifications if a large number of degraded or defective steam generator tubes been discovered in Rows 1 and 2 of the Salem Unit 1 steam generators due to the U-bend primary water stress corrosion cracking phenomenon.
During the Salem Unit 1 Eighth Refueling Outage, all the Row 1 tubes in all four steam generators were plugged due to indications of tangential cracking in a five (5) Row 1 tubes.
In addition, ten (10) tubes were plugged due to measured tube wall degradations greater than or equal to 40% and 19 tubes were plugged with tube wall degradation measured between 30% and 40%.
Due to the total number of defective tubes discovered and their locations, increased eddy current sample sizes were based on Technical Specification requirements and the alternate sampling method was not needed.
As such, PSE&G requests that our request for a license change request for the Salem Unit 1 Technical Specifications dated February 9, 1989 be formally withdrawn.
However, this phenomenon may occur in Row 2 tubes and may require resubmittal of this LCR during some future outage.
