ENCLOSURE 4 SALEM UNIT 2 -
STEAM GENERATOR EDDY CURRENT EXAMINATIONS
- Technical Specification 4.4.6.5.b - Steam Generator Tube Eddy Current Inspection.
Examinations were performed to find leaking tubes in Nos. 22 and 24 Steam Generators, Tech. Spec. examinations and anti-vibration bar contact analysis in #22 Steam Generator (North Anna concerns).
Examinations.were conducted by Westinghouse NSS and coordinated by the Salem ISI Group.
Examinations in Nos. 22 and 24 Steam Generator revealed evidence of, tangential cracking in the Row 1 tubes.
An emergency license change was.submitted to the NRC to examine all Row 1 and 2 tubes in all four (4) steam Generators.
In addition, due to industry experience, it was decided to plug all the tubes in Row 1 as well as any Row 2 tubes with indications of tangential cracking.
The License Change became Amendment #63 to Facility ?perating License No. DPR-75.
Two tubes in #24 Steam Generator required the installation of stabilizers as determined from the analysis of previous eddy current data.
The stabilizers were installed to satisfy North Anna concerns.
Both tubes were mechanically plugged after the installation of the stabilizers.
RESULTS OF THE EXAMINATIONS ARE AS FOLLOWS:

NLR-N89041 In the Steam Generator's 21-24, the first five (5) and the last five (5) tubes in row 1 were mechanically plugged because of a generic wearing problem on the tubes, due to the vibration 9f the tube lane blocking device.
