SALEM GENERATI~G STATION LICENSE NO. DPR-70 DOCKET NO. 50-272 UNIT NO. 1 April 21, 1988 TECHNICAL SPECIFICATION SURVEILLANCE NOTIFICATION REQUIREMENT 4.4.5.5.a On April 9, 1988, one tube was plugged in Unit 1 No. 13 Steam Generator.
The tube, located at row 1/column 78, was plugged due to identified primary to secondary side leakage.
Also, a leaking explosive plug was discovered at row 1/column 5 on the hot leg side.
The leaking plug was drilled out and the tube was verified intact ~sing a "helium sniff.
Upon completion of the helium sniff the tube was re-plugged using seal welding.
This notification is being mad~ as required by Technical Specification ~urveillance 4.~.5.5.a. This report is being made within fifteen days after tube plugging as specified by the surveillance.
