SALEM GENERATING STATION LICENSE NO. DPR-75 DOCKET NO. 50-311 UNIT NO. 2 SPECIAL REPORT 88-3 This Special Report addresses the circlimstances surrounding four missed hourly roving fire watches. These fire watches are required by PSE&G Safety Evaluation S-C-M200-NSE-0709-2, "Justification For Continued Operation of Salem Units 1 & 2 With Outstanding Fire Protection Concerns".

PLANT IDENTIFICATION:

Salem Generating Station - Unit 2 Public Service Electric & Gas Company Hancock's Bridge, New Jersey 08038 Westinghouse - Pressurized Water Reactor IDENTIFICATION OF OCCURRENCE:

Fire watch inspection not completed due to personnel error Event Date: 03/08/88 Report Date: 03/29/88 This report was initiated by Incident Report No.88-096.

CONDITIONS PRIOR TO OCCURRENCE: -'

Unit 1 - Mode 1 Reactor Power 100% - Unit Load 1160 MWe Unit 2 - Mode 1 Reactor Power 100% - Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:

On March 9, 1988 it was discovered that the March 8, 1988 "1620 hours0.0188 days <br />0.45 hours <br />0.00268 weeks <br />6.1641e-4 months <br />" hourly roving fire watch was not completed for the 84' Elevation Switchgear Room (Fire Area 2FA-AB-84A). Also, the March 8, 1988 "1745, 1845 and 1945 hours0.0225 days <br />0.54 hours <br />0.00322 weeks <br />7.400725e-4 months <br />" hourly roving.fire watches for Service Water Bays 3 (Salem Unit 1) and 4 (Salem Unit 2) (Fire Area 12FA-SW-90A) were not completed.

The missed roving fire watches are required to support the commitments made to the Nuclear Regulatory Commission as addressed by PSE&G Safety Evaluation S-C-M200-NSE-0709-2. The evaluation addresses Code of Federal Regulations 10CFR 50 Appendix R/Breaker Coordination concerns.

APPARENT CAUSE OF OCCURRENCE:

The root cause of this event has been attributed to personnel error.

A review of the Security System Personnel Transaction List of card reader usage revealed that one Fire Watch individual had not completed his fire watch duties as assigned. It could not be positively determined why the individual missed the particular assigned areas.

The review of the transaction list was conducted due to concerns by Nuclear Fire.& Safety Department supervision the individual involved may not have been completing the assigned fire watch tour of inspection.

ANALYSIS OF OCCURRENCE:

PSE&G Safety Evaluation S-C-M200-NSE-0709-2 addresses Code of Federal Regulations 10CFR 50 Appendix R/Breaker Coordination concerns, including the justification for continued operation of both Salem Unit 1 and Unit 2.

The fire detection equipment for the areas of concern remained operable during the periods when the roving fire watch failed to

Further review of the Security System Transaction List of card reader usage revealed that this event was isolated. The hourly roving fire watch for the missed areas was completed before and after the missed time periods. This event did not affect the health or safety of the public.

CORRECTIVE ACTION:

The individual involved was removed from the Salem site and his employment was terminated by the contractor firm the individual worked for.

This event has been reviewed by Nuclear Fire and Safety Department management with fire watch personnel. The significance of accurately following assigned fire watch duties wa~ stressed
