STATUS OF CORRECTIVE ACTIONS DEVELOPED IN RESPONSE TO THE AUGUST 26, 1986 REACTOR TRIP AT SALEM UNIT NO. 2 SALEM GENERATING STATION UNIT NOS. l AND 2 DOCKET NOS. 50-272 AND 50-311 Public Service Electric and Gas Company (PSE&G) committed in its letter of August 31, 1986, to provide monthly updates on the status of corrective actions being taken as a result of the August 26, 1986 reactor trip at Salem Unit No. 2.
To that end, to this letter provides a detailed summary of the status of each immediate *and long term cQrrective action identified in our submittal. to this letter is a copy of Safety Evaluation S-C-El30-NSE-0458, which has been revised to incorporate comments received from members of your staff.
Additionally, we would like to make a clarification regarding the authorship of the report which was used to establish the design-basis electrical loading for the station.
In our previous submittal we referred to this report as the "EG&G report".
The actual report which we are referring to is a report prepared by PSE&G.
The PSE&G report was reviewed by EG&G for the NRC and accepted by the NRC on the basis of that review.
It is our position that the previous misstatement would not have influenced your staff's decision relative to the restart of Salem Unit No. 2 and this clarification only serves to correct the public record.

ATTACHMENT 1 STATUS OF PSE&G CORRECTIVE ACTIONS SEPTEMBER 30, 1986 Immediate Corrective Action
- 1.
Station electrical bus loadings will be reduced to levels consistent with those used in the Salem degraded grid voltage study which was performed by PSE&G, and reviewed by EG&G, Idaho, Inc. on behalf of the NRC.
This condition shall be maintained until Item 1 under Long Term Corrective Actions are complete.
STATUS:
Station electrical loads were reduced prior to restart of Salem Unit No. 2.
When both Salem Units are in operation unit loads are administratively limited to approximately 82% reactor power based on station electrical loads.
- 2.
Procedures will be generated or revised as necessary to administratively control equipment removed from service in order to reach the reduced bus load levels.
STATUS:
The following procedures and directives were revised prior to the restart of Salem.Unit No. 2:
OD-23, OD-22, IOP-3, IOP-4, IOP-8

Appropriate evaluations will be performed in accordance with the provisions of 10 CFR 50.59 to justify continued operation.
STATUS:
Safety Evaluation S-C-El30-NSE-0458 was issued on August 31, 1986.
This Safety Evaluation has been revised to address comments received from members of your staff.
The revised Safety Evaluation is included as Attachment 2 to this letter.
- 4.
The station electrical lineup will be shifted such that all station electrical loads are powered from the Station Power Transformers.
STATUS:
This lineup was established prior to the restart of Salem Unit No. 2 and has been maintained since that time.
Salem Unit No. 1 was operating in this lineup due to the loss of the No. 1 Auxiliary Power Transformer.
Long Term Corrective Actions
- 1.
PSE&G shall conduct a comprehensive design analysis of the Salem Generating Station electrical distribution system.
The analysis will include consideration of transients in accordance with appropriate regulatory criteria.
STATUS:
PSE&G has retained PTI (Power Technology, Inc.) to perform the detailed analysis of the Salem Generating Station electrical distribution system.
A final report should be available by November 15, 1986.
By December 15, 1986, PSE&G will evaluate the report and prepare a final action plan based on the conclusions of the report.
- 2.
An analysis of the present protective relaying and breaker scheme will be performed to determine the root cause of the vital bus multiple transfer phenomena and establish corrective actions to preclude recurrence.
STATUS:
This analysis will be accomplished subsequent to th~ issuance of a final report by PTI.
Any corrective action will be factored into the final action plan discussed in Item 1 above.
- 3.
Updated status reports shall be forwarded to the Regional Administrator on a monthly basis until such time as all corrective actions are complete.
STATUS:
This letter provides the first monthly update as committed.
Further updates will continue to be forwarded on a monthly basis.
