RESPONSE TO NOTICE OF VIOLATION NRC COMBINED INSPECTION REPORT NO. 50-272/87-11-01 AND 50-311/87-14-01 SALEM GENERATING STATION UNIT NOS. 1 AND 2 DOCKET NOS. 50-272/50-311 Public Service Electric and Gas Company (PSE&G) has received the Notice of Violation dated May 11, 1987, regarding the failure to perform preventive maintenance on safety related motors in storage in a timely manner.
Pursuant to the requirements of 10 CFR 2.201, our response to this Notice of Violation is provided in the attachment to this letter.

L PUBLIC SERVICE ELECTRIC AND GAS COMPANY SALEM GENERATING STATION UNIT NOS. 1 AND 2 RESPONSE TO NOTICE OF VIOLATION Your letter of May 11, 1987, transmitted a Notice of Violation concerning the preventive maintenance for the following safety related motors in storage.
32-UP06 32-UP07 32-UP12 32-UP18 Auxiliary Building Exhaust Fan Motor Auxiliary Building Supply Fan Motor Auxiliary Feedwater Pump Motor Residual Heat Removal Pump Motor Maintenance Procedure MP3.5 required that each motor's shaft be rotated monthly and each motor's insulation resistance be checked in January and July.
Contrary to this requirement, during the period between July 1986 and March 1987, the required preventive maintenance was not performed.
During this period, a transition of responsibilities for maintenance of equipment in storage was in process within PSE&G.
Due to the volume of work being transferred, the work order (86-08-12-021-6) for preventive maintenance of the equipment in question was misplaced and became overdue.

PSE&G DOES NOT DISPUTE THIS VIOLATION.
- 2.
THE ROOT CAUSE OF THIS VIOLATION WAS INADEQUATE COORDINATION BETWEEN DEPARTMENTS DURING THE TRANSITION OF RESPONSIBILITIES RESULTING IN THE MISPLACEMENT OF THE PREVENTIVE MAINTENANCE WORK ORDER.

IMMEDIATE CORRECTIVE ACTIONS AND RESULTS ACHIEVED:
- a.
The required preventive maintenance activities for the above four components were completed in March 1987 and have subsequently been completed in accordance with the established preventive maintenance program.
No unsatisfactory results have been identified.
- b.
The preventive maintenance program for equipment in storage involved in the transition of responsibilities has been reviewed for other noncompliances.
Seven additional components have been identified which did not have preventive maintenance performed in accordance with the established program.
The preventive maintenance for these seven components was also identified on the aforementioned work order, and these additional occurrences can be attributed to the same misplacement.
The required activities for these were completed in March 1987 and have subsequently been completed in accordance with the established preventive maintenance program.
Three components did not meet the acceptance criteria in that the motor shafts could not be rotated.
An engineering evaluation was performed to identify necessary corrective actions.
The corrective actions are expected to be complete by July 1, 1987.
- c.
An engineering review is being performed to evaluate the impact of the July 1986 to March 1987 lapse in preventive maintenance on all eleven components.
Corrective action will be taken based on the results of the review.

LONG TERM CORRECTIVE ACTIONS TO AVOID FURTHER VIOLATIONS:
- a.
The transition of responsibilities has been completed.
- b.
The preventive maintenance program for spare parts has been reviewed and enhanced to allow more detailed scheduling capabilities for individual work tasks on each stored component.
- c.
Equipment identification has been made more specific to assure consistency between the work order and warehouse folio data bases.
This will result in an upgraded equipment history file.
- d.
Management tools such as weekly tracking reports and review,meetings have been implemented to increase the visibility of preventive maintenance activities.
- e.
Individuals are now dedicated to the scheduling of work with an increased emphasis on meeting preventive maintenance due dates.

FULL COMPLIANCE HAS BEEN ACHIEVED.
