RESPONSE TO NOTICE OF VIOLATION NRC INSPECTION REPORT NO. 50-311/87-11-01 SALEM GENERATING STATION UNIT NO. 2 DOCKET NO. 50-311 Public S~rvice Electric and Gas Company (PSE&G) has received the Notice of Violation dated April 29, 1987 regarding the missed fuel handling crane surveillance.

Pursuant to the requirements of 10 CFR 2.201, our response to this Notice of Violation is provided in the attachment to this letter.

ATTACHMENT PUBLIC SERVICE ELECTRIC AND GAS COMPANY SALEM GENERATING STATION UNIT NO. 2 RESPONSE TO NOTICE OF VIOLATION Your letter of April 29, 1987 transmitted a Notice of Violation concerning the performance of Technical Specification Surveillance 4.9.7.

This surveillance requires that the overload cutoff, which prevents crane travel with loads in excess of 2500 pounds over fuel assemblies, be demonstrated operable within 7 days prior to crane use and at least once per 7 days thereafter during crane operation.

Contrary to this requirement, on two occasions, November 17, 1986 and February 28, 1987, the overload cutoff surveillance test was not conducted within 7 days prior to crane use in operations over the spent fuel pool.

During these occurrences, the implementing procedure that directed the performance of Technical Specification Surveillance 4.9.7 was FP-PNJ-FE-1, "Fu~l Inspection Procedure."

Prior to the first occurrence, this procedure referred to the surveillance requirement in a general statement that required lifting devices to be tested in accordance with utility requirements.

In response to the first occurrence, the surveillance requirement was incorporated more clearly into the procedure by stating specifically that the spent fuel handling crane should be load tested every 7 days for the duration of its use.

PSE&G DOES NOT DISPUTE THIS VIOLATION.

THE ROOT CAUSE OF THIS VIOLATION WAS PERSONNEL ERROR.

3.

IMMEDIATE CORRECTIVE ACTIONS AND RESULTS ACHIEVED:

a.

On both occasions, the crane was placed in *a safe condition upon discovery of the failure to adhere to Technical Specification Surveillance 4.9.7.

b.

On both occasions, Technical Specification Surveillance 4.9.7 was performed the same day with satisfactory results.

c.

On both occasions PSE&G employed the same vendor to perform fuel handling activities.

Following each occurrence the individuals were counseled concerning the implications of their actions and the need for strict compliance with the procedural requirements.

Subsequent to the second missed surveillance on February 26, 1987, a letter was sent from the Station General Manager to the Vendor, describing these incidents and the necessity for corrective. actions.

In response, the Vendor has committed to review both of the occurrences with all coordinators assigned to fuel handling services and in the future with those coordinators assigned to Salem Station prior to their arrival on-site.

LONG TERM CORRECTIVE ACTIONS TO AVOID FURTHER VIOLATIONS:

a.

A sign has been posted near the crane controls to require personnel to ensure the overload cutoff surveillance test is performed as required, prio~ to use.

FULL COMPLIANCE HAS BEEN ACHIEVED.
