The root cause for this event was attributed to failure of the licensee to effectively disable a set of low-temperature overpressure protection (LTOP) relief valves. Accidental depressurization of the RCS is an analyzed condition in Salem's safety analysis report, and it is concluded that adequate core protection was maintained during this event.
However, the study found that control circuitry for gate valves that permit valve reversal during its stroke is undesirable, and can result in valve failures that can lead to system upsets and degradation of safety systems functions.
Corrective actions taken by the licensee and Region I ~'Vere judged to be appropriate.
Thus, no further action by this office is deemed necessary.
