This report is submitted in accordance with the requirements o

to Operating License DPR-49, Specifications 3.3.1.B.3 and 5.4.2.1.

Problem

On';November27 and 28, 1976, daily sensor checks on, Radiation Elements 4116A and B'ere not performed in accordance w'ith the requir'ed surveillance test procedure.

Investi ation Environmental Surveillance Test Procedure E33B004 requires that the offgas stack radiation monitor sensors be checked daily for operability.

The

=procedure requires exposure of the radiation elements to check sources and verification of an upscale response on the corresponding panel meter.

The radiation protection technician performing the surveillance test on the above dates performed a channel comparison only and failed to insert the check sources as required by procedure.

Corrective Action The technician involved has been reinstructed and placed under the supervision of a journeyman technician pending an evaluation of the technicians performance.

The evaluation willverify that. the technician has the ability'and knowledge to adequately perform all the technical duties required by h'is position.

This report was reviewed and approved by the DAEC Operations Committee and Safety Committee.
