Title:

IMPINGEMENT OF AQUATIC ORGANISMS A.

Descri tion, Anal sis and Evaluation On December 23,

1976, a special surveillance scheduled for that day was not taken, and there-fore was missed.

Specification 4.2, Impingement of Aquatic Organisms, requires that intake screens washings be examined for a consecutive twenty-four hour period, twice a week.

The first surveillance for that week was done on December 20, but the second

one, on December 23, was missed.

December 23, 1976 was observed by Florida Power Light Company as a holiday, along with December 24.

Maintenance personnel were not scheduled to work on the holiday to dump the Basket with the washings.

When personnel responsible for the surveillance arrived at the site and found no maintenance personnel, they assumed that, the surveillance had been rescheduled for the following week, unaware of what the frequency requirements were.

Even though procedures exist that tell personnel how often this surveillance must be met, the surveillance was missed.

The above reasons coupled with some communications break-

downs, caused us to miss a Technical Specification Surveillance.

One group failed to schedule the required maintenance personnel, and a second group failed to inform the first group that there were no maintenance personnel at the site.

Because of this communication breakdown, the missed surveillance was not recognized until Monday, December 27, 1976.

B.

Corrective-."Act,ion Personnel involved in this program have been instructed to familiarize themselves with the required procedures to insure that any time a surveillance is not performed as scheduled, that the proper personnel be contacted as per procedure in order to initiate corrective action as soon as possible.

Scheduling mechanisms have been implemented to provide follow-up, as an added(insurance in case a similar situation develops,"and it, can be avoided.
