INTRODUCTION Florida Power 6 Light Co.
(FPL) experienced an electrical power system disturbance on Sunday, May 14, 1978, which resulted in the loss of po~er to an area of the FPL system.
(see Fig.
1)
This blacked out area included the loss of off-site power to St. Lucie Plant for approximately 8 minutes.
Fault recorder records, the West Palm Beach computer printout, and logs were used to reconstruct the sequence of events.
PRE-DISTURBANCE CONDITIONS Only conditions in affected area are listed:
1 St. Lucie Plant unit No.
1 off for refueling.
2 Pratt 6 Whitney 240kV ring bus open (240G36835 open) prior to switching.
This switch had been previously reported to be a cing.
The Dispatcher believed the switch to still be inoperative.
3 Two 240/138kV autotransformers (49-1035
& 49-1168) at Midway Substation were recently installed to replace smaller capacity units.
DISTURBANCE On Sunday, May 14, 1978, at approximately 7:45AM while switching out the Pratt a Whitney-Ranch 240kV circuit for a routine clearance, a condition was created on the Pratt 6 Whitney south 240kV bus that caused lightning arresters on this bus to fail destructively.
(see Addendum p.l)
Hot ionized gas from the lightning arresters caused a "C" phase to ground fault on the Midway-Ranch 240kV circuit which passes directly over the Pratt 6
Whitney bus.
(see Fig.
3)
At Midway Substation improper connection of the relay current polarizing*
circuit resulted in incorrect operation of the Directional Carrier Ground Relays (CLPG) on the Malabar-Midway 240kV No. 1, Malabar-Midway 240kV No.
2 and the Midway-Plumosus 138kV circuits.
Erroneous current polarizing like-wise resulted in failure to trip by the ground relays of the Midway terminal of the faulted Midway-Ranch 240kV circuit.
Failure of this terminal to trip resulted in a back-up operation of the autotransformer tertiary relay at Midway Substation.
The Hartman-Midway and Okeechobee-Midway lines then tripped interrupting service to St. Lucie and the stations shown in Figure 1.
Diesel generators at St. Lucie started and the station se vice automatically transferred.
St. Lucie remained on diesels until after system conditions stabilized.

For given phase to ground fault, the north autotransformer tertiary relay is more sensitive than the south autotransformer tertiary relay.
(see addendum P 3.)
This accounts for the operation of only the north autotransformer tertiary relay.
The two main contributing factors to the power loss were:
1 Improper switching at Pratt 6 Whitney Substation.
2 - Improper connection of both 240/138kV autotransformer polarizing CT circuits at Midway Substation.
RESTORATION Several attempts were made to energize the north bus at Midway Substation via the Malabar 240kV No.
2 circuit. 'owever, the lockout relay previously trip-ped by the tertiary relay had not been eset.
The south bus at Midway Sub-station was energized via the Ranch 240kV circuit.
As switching continued to normalize the system, a breaker at Pratt
& Whitney was closed and the original lightning arrester problem was recreated.
This resulted in the deenergizing of Midway Substation.
Final reenergization of Midway Substation and St. Lucie Plant was accomplished by the closing of the Ranch 240kV circuit at 7:53:30.
This resulted in the loss of off-site power to St. Lucie Plant for approximately eight minutes and sixteen and one-half seconds.
Switching was then carried out until the entire affected area was returned to normal with the exception of Midway Substation's north autotransformer.
CORRECTIVE ACTIONS 1 The improper polarizing at Midway Substation has been corrected.
2 The north and south buses at Pratt
& Whitney Substation have been tied together to avoid the bus high voltage problem encountered during switching.
3 Drawings j.n use at the time were reviewed and an error was detected.
As a result of this, System Protection Memorandum No. 3.10.1-A titled "The Connection of CT's In An Autotransformer When Used As a Source For Ground Polarizing Circuit" was issued May 22, 1978.
This memorandum outlines the procedures for checking transformer nameplate data against FPL drawings as well as the electrical tests which are to be made.
This memorandum will be incorporated into the Power Supply Procedures Manual and will prevent a
reoccurence of this event.

ADDENDUM TO DISTURBANCE REPORT
LIGHTHI NG ARRESTER FAI LURE The switching at Pratt 6 Whitney Substation left the 240kV bus energized with no ground source.
The neutral was free to wander according to unbalanced stray impedances to ground.
The arresters are rated at.192kV 8-G and are expected to conduct at 1.25 times the rated voltage, or 240kV.
A fully displaced neutral would subject two of a set of three arresters to 240 x ~2 (peak to RHS) or 339kV.
The resulting unbalanced voltages caused continuous conduction in four arresters, 2 on the bus and 2 on the transformer, resulting in thermal failure.
Fire was reported by the operator at Pratt 6 Whitney to continue for an extended period; enough time for him to report to the Dispatcher and then cross the switchyard and operate a switch.

POLARIZING THEORY Directional ground relays, such as the type CLPG and JBCG, require that a reference quantity be established in order for the relay to determine the direction of the current flow at the relay location.
This reference quantity is referred to as the polarizing quantity and for directional ground relaying it may be either zero sequence current or voltage.
It is against this reference th'at an operating quantity is compared.
The operating quantity will in all cases be proportional to and derived from the iine current at the relay location.
Since line currents will reverse as a function of the fault location, it is imperative that the polarizing quantity remain fixed in direction if the relay is to operate correctly.
In transmission stations where there is a wye-grounded autotransformer with delta tertiary, the tertiary is almost invariably used as a source of polar-izing quantity (viz., current).
Since the relay receives its polarizing current via a current transformer(s) in the tertiary, it is also imperative that careful attention be given to appropriate current transformer polarity and relay stud connections, else the relay may look in the opposite direction of that intended and operate (unsupervised, in the case of a type CLPG relay) for any phase-to-ground fault within its sensitivity range.
Ground Distance relays, such as the type CEYG, are potential polarized and would remain unaffected by errors in the station current polarizing circuit.

CRITERIA FOR SETTING TERTIARY RELAYS TAP: Based on 50% of the Tertiary current for the minimum End Fault on a circuit connected to either the high-voltage or low-voltage side of the autotransformer to be protected.
DIAL: Based on 42 cycle delay for the high-voltage or low-voltage Bus Fault which yields the maximum Tertiary current.
FIND: The Tertiary currents of the Midway substation autotransformers 49-1035 (North) and 49-1168(South) to be used in setting their Tertiary Relays per the
'bove bases.
