COMBINED INSPECTION 50-272/82-05 AND 50-311/82-08 SALEM GENERATING STATION UNITS NO. 1 AND 2 MARCH 9 THROUGH APRIL 5, 1982 We have reviewed the item of violation identified in your letter, dated April 9, 1982.
The following is our response to that item:
ITEM OF VIOLATION A.l Technical Specification 6.8.l and Regulatory Guide 1.33 require implementation of procedures for safety related tagging.
Licensee procedure AP-15, Safety Tagging Program, requires the work supervisor to ensure the equipment is isolated completely and that all tags are properly pl'aced before starting work.
Contrary to the above:
On March 15, 1982, a tag issued to isolate valve 1RH26 in Unit 1 was found on the breaker for valve 2RH26 in Unit 2, with the valve motor de-energized.
On March 26, 1982, Safety Injection Pump 12 was isolated by tagging the motor breakers for valves 12SJ33 and 11SJ45.
No tags were placed on the Limitorque manual operators to preclude local opening of the valves.
R,eply to* Ttem A.1 An investigation was conducted concerning the inadvertent tagging of 2RH26 instead of 1RH26.
It disclosed that the tag was placed on the wrong 230V breaker due to operator error.
The work super-visor did not have an opportunity to verify tag placement because
Mr. Richard W. Starostecki, Director U.S. Nuclear Regulatory Commission 5/7/82 the NRC Resident Inspector discovered the error after the place-ment of the tag, but before actual work was to begin.
The operator involved was counseled on the importance of careful tagging compliance.
Due to a misunderstanding between two station departments, the
.manual operators for valves 12SJ33 and 11SJ45 were left untagged.
The work supervisor did not check for tags on the manual operators.
To preclude recurrence of the problem, all maintenance supervisors will attend classes on proper tagging procedures and verifications.
