ATTACHMENT 3 TO LCR 82-04 SAFETY EVALUATION The change to paragraph 4.4.10.1.2 will not involve an unresolved safety question for the following reason:
- 1.
Remote visual examination is being replaced by direct visual examination.
Direct visual examination is considered to be at least equal to and normally superior to visual examination by remote means.
Video scanning was employed initially as a substitute for direct visual only for personnel protection, not for image enhancement potential.
- 2.
Videotape recordings were proposed initially for comparison purposes only.
In the absence of record-able lndications, they serve no more use than a written report of visual examination.
- 3.
Visual examination was initially proposed only as a supplement to ultrasonic examination fro~ the external surface, to provide information in the event cracking progresses to a point where it becomes visible to the unaided ey~.
This has not happened yet, and ultra-sonic examination has shown no indication of growth of flaws existing within the cladding.
Since cladding thicikness is not factored into the design of the vessel pressure boundary, flaws restricted to the cladding may be tolerated as suggested by ASME Code Case N-118 and deletion of examination requirements from current ASME Section XI editions.
ATTACHMENT l TO LCR 82-04 DESCRIPTION OF CHANGE A change to paragraph 4.4.10.l.2 of the Technical Specifi-cation is requested.
The present wording of the paragraph is as follows:
4.4.10.l.2 Augmented Inservice Inspection Program for Stearn Generator Channel Heads -
The steam generator channel heads shall be ultrasonic inspected during each of the first three refueling outages using the same ultrasonic inspection procedures and equipment used to generate the baseline data.
These inservice ultrasonic inspections shall verify that the cracks observed in the stainless steel cladding prior to operation have not propagated into the base material.
The stainless steel clad surfaces of the steam generator channel heads shall also be 100%
visually inspected during the above outages and a tele-vision camera shall be used to make a videotape recording of the condition of this cladding.
Each videotape shall be compared with those obtained during.the previous outages to determine that the cladding does not show any ahnormal degradation.
The proposed revision is as follows:
4.4.10.1.2 Augmented Inservice Inspection PrograM for Steam Generator Channel Heads -
The steam generator channel heads shall be ultrasonically inspected during each of the first three refueling outages using the same ultrasonic inspection procedures and equipment used to generate the baseline data.
These inservice ultrasonic inspections shall verify that* the cracks observed in the stainless steel cladding prior to operation have not propagated into the base material.
The stainless steel clad surfaces of the steam generator channel heads shall also be visually inspected during the above outages.
This may be accomplished by direct visual examination or by remote means such as televison camera.
If the visual examination, either direct or remote, reveals detectable cladding indi-cations, a record shall be made by means of a video tape recording or photographs for comparison purposes.
-it ATTACHMENT 2 TO LCR 82-64 REASON FOR CHANGE
- A remotely operated.video scanning system was developed specifically to provide a means of ~isually examining the interior clad surfaces of the Salem steam generator channel heads as*a supplement to ultrasonic examination from the exterior surfaces.
Remote video scanning.was proposeq as an alternate to direct visual examination to minimize radiation exposure to inspection personnel.
Experience at Salem Unit No. 1 during the first two refueling outages has demonstrated to the contrary; m6re exposure w~s incurred in setting up, adjusting and removing the equipment than would have been_incurred by a simple direct visual examination.
Furthermore, cracking known to exist in the cladding surface ~s determined by dye penetrant examination has not been detectable so far by either direct or remote visual examination; thus negating the value of, and need for, a*
videotape record.
