Technical Inspection Branch, Division of Engineering and Technical Inspection NRC INSPECTION 50-272/81-11 SALEM GENERATING STATION UNIT NO. 1 MARCH 31, 1981 We have reviewed the report of your inspection conducted by Mr.
- w. M. Grayson of the Region V Office on March 31, 1981.
The inspection was performed on a shipment of radioactive waste shipped from our facility to the U.S. Ecology, Incorporated burial site at Beatty, Nevada.
Our response to the items of noncompliance identified in Appendix A are as follows:
Violatio*n A 10 CFR 71.5 prohibits delivery of licensed material to a carrier for transport unless the licensee complies with applicable regulations of the Department of Transportation in 49 CFR Parts 170-189.
49 CFR 173.393(n) (3), states, "Each closure device of the packaging, including any required gasket, is properly installed and secured and fre~ of defects."
- Contrary to the above, the bolt to Drum No. 1926, that was supposed to secure th~ package, was not in the lock ring of the drum as observed by the inspector on March 31, 1981, as the drums were being unloaded.
The bolt was lying on the floor of the truck.

Grier, Director 7/29/81 Office of Inspection and Enforcement Reply To Item A An investigation of the event indicated the apparent cause of the drum lid clamping ring bolt falling out was due to the banding of the drums to a pallet, thus removing the lid clamping ring tension from the bolt.
The removal of this tension allowed the bolt to vibrate out of the clamping ring.
The bolt was reinstalled on the clamping ring to secure the drum lid.
Station Procedure RP-13.004 has been revised to include a requirement to destroy the clamping ring bolt threads.
This action will prevent the bolt from vibrating out or coming loose from the clamping ring.
We are in compliance now.
Violation B 10 CFR 71.5 prohibits delivery of licensed material to a carrier for transport unless the licensee complies with applicable regulations of the Department of Transportation in 49 CFR Parts 170-189.
49 CFR 177.842(d), states, "Packages must be so blocked and braced that they cannot change position during conditions normally incident to transportation."
Contrary to the above, on March 31, 1981, the inspector observed that Drum No. 1900 had moved against the door of the trailer such that on opening the door of the trailer, Drum No. 1900 came out of the trailer and hung suspended by a metal band.
Reply To Item B Investigations revealed the cause of the inadequate bracing of the load to be a failure of a wooden 4"X4" secured to each side of the trailer at the rear of the load which was providing the required bracing.
Station Procedure RP-7.004 has been modified to show all bracing points.which include both lateral and axial bracing at the trailer.
This action should prevent such occurrences in the future.
We are in compliance now.
