ADDENDUM TO NOTE ON SALEM 2 SSER 5 (DRAFT)

In addition to the issues brought to your attention in my December 15, 1980 note, Section 6.2.3.l should also be examined carefully because Salem 2 was granted a temporary exemption from the requirements _of GDC 57 "Closed System Isolation Valves" as discussed in that section.

That section was based upon draft input received from the Containment Systems Branch.

The enclosed Supplemental Safety Evaluation Report (SSER) was prepared by DE:MQE, Equipment Qualification Branch. This SSER supplements the information contained in Section 3.11 of Supplement Numbers 3 and 4 to the Safety Evaluation Report for the Salem Unit No. 2 Nuclear Station.

This supplement provides information relating to the degree to which the environ-menta*1 qualification program for the applicable electrical equipment at the Salem Unit No. 2 plant complies with the staff's position described in NUREG-0588, "Interim Staff Position on Environmental Qualification of Safety Related Electrical Equipment.

11 This.is an area of concern which was identified in a letter dated February 21, 1980 from D. F. Ross, Jr., Acting Director, Division of Project Management, to Operating License Applicants. 3.11 Environmental Qualification for Safety Related Electrical Equipment In December 1979 the staff issued guidance for the environmental qualifi-cation of safety related electrical equipment (~UREG-0588, "Interim Staff Position on Environmental Qualification of Safety-Related Electrical Equipment 11 ).

By letter dated February 19, 1980 the staff requested that the*applicant review the environ~ental qualification documentation for each item of safety-related electrical equipment which could be exposed to a harsh environment and to identify the degree to which the associated environmental qualifjcation program complies with the staff's position as d~scribed in the NUREG.

Where there are deviations we requested the appli-cant to provide the basis for concluding that the associated environmental qualification program demonstrates that each item in question is environ-mentally qualified for its service conditions.

In response to this request the applicant provided an environmental qualification submittal on August 26, 1980 which provides the results of the applicant's review.

The staff performed an audit, on September 8-9, 1980, of the applicant's test data and/or documentation referenced in the submittal.

  • The staff did not arrive at the same conclusion that the applicant did on two out of the four items identified as environmentally qualified.

In addition, short-comings were identified in the QA aspects of the program.

In an attempt to resolve open items identified at the audit the licensee provided clarifying information to the staff on September 18 and October 1, 1980.

Several conference calls between the staff and PSE&G were held to resolve staff

    • concerns.

On October 30, 1980 the applicant was requested to attend a meeting with the staff to discuss the adequacy of PSE&G's environmental qualification program.

The meeting resulted in a request that.PSE&G reevaluate the environmental qualification of safety related electrical equipment and provide a revised submittal.

In -response to this request, the applicant provided an environmental qualification submittal on December 1, 1980.

The results of this re-review are as follows.

The applicant identified*twenty-eight out of sixty item types, necessary to mitigate the consequences of an accident, as being deficient in meeting the requirements of NUREG-0588.

The deficiencies were primarily indicated as lacking documentation.

The licensee provided interim and final resolu-tions for all of these.

The. resolutions consisted of analyses of actual service requirements, replacement, relocation to a non-harsh environment, design and procedural changes.

The staff is not in total agreement as to the finality of some of these resolutions. However, the corrective course being pursued by the applicant is considered adequate for the interim period and therefore provides sufficient basis for operation of the S~lem 2 unit atthe 100% ~ower leVel.

Final resolution by the staff will be addressed in the February 1, 1981 Safety Evaluation Report.

On December 9-10, 1980 the staff performed a preliminary audit of the test data and/or documentation referenced in the applicant's December 1, 1980 submittal. The audit included a re-review of four items previously reviewed in the September 8-9, 1980 audit and review of three additional items identified as environmentally qualified by the applicant.

  • The staff's December 9-10, 1980 audit of previously reviewed items revealed that radiation qualification documentation was lacking for the slide bearing lubricant on a Westinghouse chargfng pump motors.

The applicant provided confirmatory information from Westinghouse on the adequacy of this lubricant.

The documentation supporting the environmental qualification of the three new items was found to be satisfactory. The applicant identified a gasket failure during the test of a NAMCO limit ~witch. Although the limit switch continued to function throughout the test, the applicant plans to replace the gasket on these limit switches with qualified gaskets at the first refueling outage.

An additional audit, in January 1981, will complete the staff's evaluation of _the applicant's environmental qualification program.

The Commissioner's Memorandum and Order dated May 23, 1980 directs the staff to complete its review of environmental qualification, including the publi-cation of the Safety Evaluation Reports, by February 1, 1981 for all operating reactors. Also, this order directs that by no later than June 30, 1982 all electrical equipment in operating reactors subject to this review be in compliance with the NUREG or operating reactor guideline documents as appropriate. The staff intends to complete the environmental qualification review in accordance with these dates.
