March 19, 1980 NRC INSPECTION 50-272/80~01 INSPECTION DATE JANUARY 22-25, 1980 SALEM NO. 1 UNIT We have reviewed the report of your inspection on January 22-25~
1980, which was transmitted with your- letter dated February 29, 1980 and received on March 5, _1980.
Our response *to the item of non-compliance in Appendix A of your Inspection Report is as follows:
The deficiency in NRC Inspection 50-272/80-01, Appendix A states:
Technical Specification 6.8.l states that written procedures shall be established and implemented for the applicable. procedures recommende~ in Appendix "A" of Regulatory Guide 1.33 and surveillance and test activi-ties qf safety related equipment.
Salem Performance Department Manual paragraph 3.4.5.5 states in. par.t "~ ** testing activities shall be verified by the. *

Salem Performance Department Manual paragraph 3.4.5.7 states in part "Satisfactory inspection re~ults shall be noted on the Procedure Data Sheet by the Quality Assurance Staff members by initialling or stamping the applicable *** data sheet.* Unsatisfactory inspection results shall be reported to the responsible senior department supervisor *.* "

3-19-80 Contrary to the above, data recorded for the cali-bration of TE-421 A/B No. 12 Reactor Coolant Loop Delta T/Tavg.
Protection Channel II, performed August 30, 1979, data for two output monitor points required to be at 3.000 vo*lts +20 millivolts were recorded as 3.988 and 3.989 volts-respectively; and data recorded for channel functional test of lPT-505 First Stage Turbine Impulse-Pressure - Channel I, performed April 30, 1979, data recorded for lBS-512 Steam Flow greater than reference flow to Safety Injection required to be at 3.169 volts +32, -22 millivolts was recorded as 2.178 volts.
Both completed procedures, which contained the above data, were reviewed and signed by a supervisor; and, in addition, one data sheet was stamped and verified as correct by a quality control inspector.
The errors were not observed nor was corrective action taken as result of the supervisory and quality control reviews.
Reply to deficiency:
- 1.
The cause was a personnel error on the part of the.Per-formance Supervisor and *QC Inspector. * *
- 2.
To correct the situation, the two procedures in question were completed again.
The results indicate that the data had been improperly recorded by the technician, during the tests in question, ratJ;ier than being out of specification~
- 3.
To prevent future items of noncompliance, all Performance-
- :i:&c Supervisors and QA staff members have been reinstructed as to the importance of an accurate data review, especially if the review is conducted at the completion of a procedure.
- 4.
