Failure to Continuously Monitor Liquid Waste Discharge CONDITIONS PRIOR ~O OCCURRENCE:
Operational Mode 5 DESCRIPTION OF OCCURRENCE:
During release of radioactive water from llCVCS monitor tank, the control operator noticed that the 1Rl8 liquid release monitor channel had failed down scale.
The release was promptly terminated.
8360 gallons of water were released during the discharge.
The time that the monitor failed is not known.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The cause of this occurrence was ground water in leakage along the back wall of elevation 84' of the Auxiliary Building.
The water ran down the wall and into the 1Rl8 detector cable connector.
The detector high voltage connections shorted and burned off one wire, opening the detector circuit.
ANALYSIS OF OCCURRENCE:
Environmental Technical Specification 2.3.2.d requires that the radio-activity in liquid wastes shall be continuously monitored and recorded during releases.
Whenever these monitors are inoperable for a period not to exceed 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, two independent samples of each tank to be discharged shall be analyzed and two plant personnel shall independently check valving prior to the discharge.
If these monitors are inoperable for a period exceeding 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, no release from a liquid waste tank shall be made and any release in progress shall be terminated.
The release was terminated as soon as the 1Rl8 monitor failure was detected.
No liquid releases. were made unti,l the monitor was returned to service.

LER 79-73/04L CORRECTIVE ACTION:
The detector cable was dried and a new connector installed.
The cable and connector have been covered with plastic.
Lead shielding has been placed around the detector to improve operation by decreasing the background radiation to the detector.
This shielding also provides some protection from high moisture conditions.
