Radiation Monitoring System Channel 1Rl8 Inoperable CONDITIONS PRIOR TO OCCURRENCE:
Refueling Mode 6 DESCRIPTION OF OCCURRENCE:
At 0720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br />, the Liquid waste Disposal RMS Monitor Channel 1Rl8 failed a routine channel calibration check.
Environmental Technical Specif i-cation 2.3.2.d action was implemented.
The channel was repaired, tested and returned to service at 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br /> at which time the ETS action was terminated.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The cause of this occurrence was a failed G.M. tube in the channel detector which caused the detector output to increase by a factor of 2.
ANALYSIS OF OCCURRENCE:
ETS 2.3.2.d requires that the radioactivity in liquid releases shall be continuously monitored and recorded during releases.
Whenever these monitors are inoperable for a period not to exceed 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, two inde-pendent samples of each tank to be discharged shall be analyzed and two plant personnel shall independently check valving prior to the discharge.
No liquid release was in progress at the time the channel was inoperable nor were any releases made during period of inoperability.
The increase in counts output failed the channel in the conservative direction.
CORRECTIVE ACTION:
The channel detector was replaced and the channel was recalibrated.
A satisfactory channel functional test verified operability.
