78-20/04L 4/4/78 2/17/78 Salem Generating Station Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:
Weekly river water samples for biocide determinations required by Environmental Technical Specification Section 3.1.1.1 were not taken during the weeks of June 6, 13, and 27, September 19 and 26, and December 19 and 26, 1977, when the service water system was being chlorinated.
The dates of the missing data were determined during a routine annual program review on.February 17, 1978, prior to submitting the Annual Nonradiological Environmental Report.
CONDITIONS PRIOR TO OCCURRENCE:
N/A DESCRIPTION OF OCCURRENCE:
Weekly samples for biocide determination were taken whenever the circulating water system was being chlorinated, but on the occasions described above, sampling was omitted when only the service water system was being chlorinated.
There was a communications misunderstanding between sampling person-nel and station personnel concerning the status of chlorination and the need for taking samples when the service water system was being chlorinated, even though the circulating water system was not.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error.
ANALYSIS OF OCCURRENCE:
No significant environmental impact is indicated since service water system flow volumes are extremely small (approximately 40,000 gpm) relative to circulating water system flow (555,000 gpm) per chlorination cycle.

CORRECTIVE ACTION:
Sampling personnel were informed of the chlorination system details during the NRC inspection and a followup meeting on February 17.
Written instructions covering station contact procedure are under preparation.
The station has designated the Senior Performance Supervisor/HP-Chemistry, as the point of contact for chlorination system status.
