The cover letter of Inspection Report 50-255/98002 noted that weaknesses were observed in operating crew communications and supervisor command and control in the control room. Specifically, the Inspection Report discussed that the positioning of the Control Room Supervisor (CRS) next to the Reactor Operator (RO) during startup activities detracted from the CRS's ability to maintain control room oversight due to being too narrowly focused on the startup activity. Additionally, the close proximity of the CRS to the RO enabled softly spoken communications to occur between these two watchstanders without acknowledgement and participation from the remaining control room crew members. Finally, the soft communications detracted from the authoritative demeanor that should be displayed by the CRS.
- The Palisades Operations Department has reviewed this observation and concurs that the CRS should be physically positioned to maintain a command presence over the entire control room and as well promote solid communication among the entire control room team. Accordingly, we have revised our standard to address this matter and communicated our expectations to the operating crews.
Assessments of control room activities during recent plant evolutions have shown strong improvement in CRS performance. We will continue to focus attention in this area via performance assessment and simulator training.
