NRC INSPECTION REPORT 50-25.5/96017(DRP)
. AND NOTICE OF VIOLATION
Dear Mr. Palmisano:
On January 10, 1997, the NRC completed an inspe_ction at your Palisades Nuclear
Generating Plant. The en~losed report presents the results of that inspection.
During this inspection period, the license~ completed a 56 day refueling outage. Following
. startup from the refueling outage, there were equipment problems that led to two forced
outages, but overall t_he inspectors viewecj the refueling outage as successful .
- Based on the results of this inspection, the NRC has determined that a violation of NRC
requfrements occurred. The violation is cited in*the enclosed Notice of Violation (Notice),
and the circumstances surrounding it are described in detail in the enclosed report. The
violation is of concern because of your staff's failure to properly maintain adequate control
- and over~ight of primary coolant system temperature.

In add~tion, we are concerned with the large number of rework items following the
refueling outage that necessitated two forced outages. We are also concerned with
adequacy of control and oversight of core operating parameters; specifically, radial peaking *
factors and primary.coolant system temperature. An unresolved item was identified
concerning adequate oversight of radial peaking factors. This item was identified by your.
staff after approximate.Iv seven hours had elapsed prior to taking appropriate action. *
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your ,respon~e. In your response, you should document
- the specific actions taken and any additional actions you plan to prevent recurrence. Your
response may reference or include previous docketed correspondence, if the
correspondence adequately addresses the required response. After reviewing your
response to this Notice, including your proposed corrective actions *and the results of
- future inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
