NRC INSPECTION REPORT 50-255/98003(DRS) AND
Dear Mr. Palmisano:
This refers to the inspection conducted on March 25 through April 10, 1998, at your
Palisades Nuclear Generating Plant. The purpose of the inspection was to follow up on
issues identified during the NRC Design Inspection conducted from September 16 through
November 14, 1997, and documented in inspection report 50-255/97201. The enclosed
report presents the results of this inspection.
While good progress had been made in addressing the individual issues identified during the
Design Inspection, six violations of NRC requirements were identified. The violations are
cited in the enclosed Notice of Violation (Notice), and the circumstances surrounding them
are described in the enclosed report. They involved procedural violations in the areas of
engineering, maintenance, and surveillance testing; failure to properly scope and include
certain valves with safety functions in the inservice test (IST) program; and two examples of
inadequate design control dating back to original plant construction. The multiple examples
of failure to adhere to procedural requirements indicate some weakness in this area. In
addition, two deviations from previous licensing commitments were identified. The deviations
are cited in the enclosed Notice of Deviation, and the circumstances surrounding them are
also described in the enclosed report.
You are required to respond to this letter and should follow the instructions specified in the
- enclosed Notice when preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
