Bordine Manager, Licensing NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM (NPDES) PERMIT VIOLATION REPORT-MAY 1996 THROUGH OCTOBER 1996 This report is being submitted in accordance with: (1) Section 3.2 of the Palisades Environmental Protection Plan (Non-Radiological) entitled, "Reporting Related to NP DES Permits and State Certifications," which states that copies of reports to the State of Michigan indicating violations of NPDES permits shall be sent to the NRC; and (2) our April 17, 1985, letter to the NRC which states that we will comply with Sections 3.2 and 5.4.2 of Appendix B of the Palisades Technical Specifications by submitting all NPDES permit violations and reports of oil, salt or polluting material loss on a semi-annual basis within 30 days from April 30 and October 31.

During the period from May 1, 1996, through October 31, 1996, no NPDES violations were identified. One report of oil, salt or polluting material loss was made. The attached NP DES Permit Violation Report also contains a summary of the recent NPDES permit activity.

SUMMARY

OF COMMITMENTS This letter contains no new commitments and no revisions to existing commitments.

NON RADIOLOGICAL NPDES PERMIT VIOLATION REPORT MAY 1, 1996 THROUGH OCTOBER 31, 1996 NPDES Notifications During this period of May 1, 1996 through October 31, 1996, there were zero NPDES notifications reported to the Michigan Department of Environmental Quality (MDEQ).

Oil. Salt. or Polluting Material Loss During this period of May 1, 1996 through October 31, 1996, there was one report of oil, salt, or polluting material loss to government agencies.

On October 23, 1996, a release of approximately one gallon of diesel fuel that was mixed with groundwater from a construction dewatering activity, was reported to the

  • Pollution Emergency Alert System (PEAS). The released material was immediately contained and cleaned up and did not threaten the environment.

The diesel fuel and water mixture was discharged onto the ground. The release was reported to a PEAS Operator at 1725 hours0.02 days <br />0.479 hours <br />0.00285 weeks <br />6.563625e-4 months <br /> on October 23, 1996.

  • The released material was immediately contained and cleaned up on the same day of discovery. No threat to the environment resulted from this incident.

  • Pursuant to the provisions of Act 245 of the Public Acts of Michigan 1929 as amended, regulations have been issued which require that all owners, managers or operators of vessels, oil storage or on land facilities shall notify the Water Resources Commis.'Sion or his authorized representative of oil, salt and polluting material losses. This notification shall be made promptly by telephone or telegraph giving briefly the particulars, and by mail, giving a detailed account of events and conditions.

This project required the retrofitting of the underground piping from the replaced underground storage tank to connect to the new underground storage tank. During the process of installing dewatering equipment (sinking points )to allow fttxr work on the piping, a slight sheen of diesel fuel on the development water being discharged on the ground was detected.

Nature of Loss (Include Complete Description of Damage)

A small amount IO.lf' dewatering equipment installation water with a sheen of diesel fuel was released onto the ground. The som'Ce of the material was in the vicinity of the former diesel fuel UST's fill pipe. There was no evidence of any releases to any drains or surface water. A small area of soil was also stained.

Additional Comments (Indndl:: !Meihod of Control, Plans for Prevention of Recurrence, Etc)

Absorbent material was immediately placed around a stormwater drain. The discharge point of the stormwater drain was inspecied to determine if any diesel fuel and water had escaped into the drain. A cleanup contractor was called an.cl. completed the cleanup of the standing water (in a.small po~l or~ the surface of the ground) and the stained soil. The ;area where the dewatering equipment was being installed was excavated. No evidence of soil and groundwater contamination was discovered.
