NRC RADIATION PROTECTION INSPECTION REPORT 50-255/96009(DRS)
Dear Mr. Palmisano:
On September 12, 1996, the NRC completed an inspection of your Palisades
Nuclear Generating Plant.
The results of this inspection were discussed on
September 12, 1996, via telephone conversations between Mr. Patrick Louden of
my staff and Messrs. Daniel Malone. and Robert Vincent of your staff. The
enclosed report presents the results of that inspection.
Areas examined during_the inspection are identified in the report. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, observations, and interviews with persorinel.
The
purpose of the inspection was to review the circumstances surrounding the
- discovery that the alarm setpoint of the continuous -air monitor for the
control room heatingr ventilation, ~nd air-conditioning (HVAC) system was
incorrectly established.
Based on the results of this inspection, the NRC
ha~ determined.that a
violation of NRC requirements occurred.
The violation is cited in th~
enclosed Notice of Violation (Notice), and the circumstances surrounding .it
are described in detail in the enclosed report.
The violation is of concern
because of the duration the alarm setpoint for the control room HVAC system
continuous air monitor was nonconservative.
Please _note that you are tequired to respond to this letter and should follow
the instructions specified in the enclosed Notice when preparing your
response.
The NRC will use your response, in part, to determine whether
further enforcement action is necessary to ensure compliance with regulatory
requirements.
In accordance with 10 CFR 2.790 cif the NRC's "Rules and Practice," a. copy of
this letter and its enclosures will ~ place in the NRC Public Document Room
(PDR) .
