- Dear Mr. Palmisano:

This refers to the routine safety inspection conducted by D. G. Passehl and
others of this office from July 4 through August 21, 1995.
The inspection
included a review of activities at your Palisades Nuclear Generating Facility.
At the conclusion of the inspection, the findings were discussed with those
members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, interviews with personnel, and observation df
activities in progress.
No *violations of NRC requirements were identified during the course of this
inspection.
However, a significant weakness that remained from the previous
inspection was in the area of control of foreign material. Specific examples
that occurred d~ring this inspectioh were the discovery of a yellow poly
radiation control bag found in the impeller of a high pressure safety
injection pump, and the inadvertent spraying of primary coolant pump oil into
the reactor cavity during decontamination activities.
In addition, foreign*
material may have played a part in the unexpected control rod motirin event
that occurred during low power physics testing at the end. of this inspection. -
We therefore consider the issue of foreign material control an unresolved
item.
Your staff agreed to respond in writing within 60 days of receipt of
this letter stating the actions you propose to improve the control of foreign
material.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosed inspection report will be placed in the NRC
Public Document Room .
