Haas Plant Safety and Licensing Director NPDES PERMIT VIOLATION REPORT - MAY 1994 THROUGH OCTOBER 1994 The attached report of NPDES Permit Violations and Oil, Salt, or Polluting Material Loss is being provided in accordance with the Palisades Environmental Protection Plan {Non-Radiological), Appendix B to the Facility Operating License.
During the period from May 1, 1994 through October 31, 1994, no NPDES violations were identified. Three reports of oil, salt or polluting material loss were made.
The attached NPDES Permit Violation Report also contains a summary of the recent NPDES Permit activity.

NON RADIOLOGICAL NPDES PERMIT VIOLATION REPORT MAY 1, 1994 TO OCTOBER 31, 1994 NPDES Notifications During this period (May 1, 1994 to October 31, 1994) there were zero NPDES notifications reported to the Michigan Department of Natural Resources (MDNR).
Oil, Salt, or Polluting Material Loss During the time period of May 1, 1994 to October 31, 1994, there were three reports of oil, salt, or polluting material loss to the MDNR.
- 1.
On August 25, 1994, there was a 5 gallon clear liquid release from the Plant's septic system.
- 2.
On August 30, 1994, there was 3 gallon clear liquid release from the Plant's septic system.
- 3.
On October 6, 1994 there was a loss of an estimated 5-18 gallons of lubricating oil from a cooling tower fan into the circulating water system.
Other Act i vites Other activities which may be of interest pertaining to water quality are summarized below:
- 1.
Continuous chlorination of the service water system was performed during the following periods, August 2, 1994 to August 15, 1994 and September 21, 1994 to September 28, 1994.
- 2.
Effective molluscicide treatments to the Service Water and the Fire Protection Systems were performed on July 12, 1994 and October 27, 1994.
Prior to the October 27th treatment, 60 cubic yards of zebra mussels were removed from the exterior and ceiling of the intake structure crib during October 10, 1994 through October 14, 1994.
- 3.
The NPDES Waste Water Discharge Permit No. MI 0001457 for the Palisades Plant was reissued by the MDNR on September 15, 1994.
The new permit becomes effective January 1, 1995 and will expire on October 1, 1998.
