NRC INSPECTION REPORT NO. 50-255/940ll(DRS)

This refers to the special safety inspection conducted by R. M. Lerch,

W. J. Kropp, D. S. Butler, J. G.

Guzman~ M. E. Parker and D. G.

P~ssehl of

this office from April 12 through July 28, 1994, of Diagn6stic Evaluation Team

(DET) issues that related to starting up the plant and to the discussion of

our findings with you and Mr. Rogers of your staff at the conclusion of the

inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspe~tion. Within these areas, the inspection coniisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

The purpose of this inspection was to evaluate specific issues identified by

the DET as potential safety issues and to determine ~hich issues must be

addressed prior to restarting the plant and whether these issues had been

adequately addressed by your staff. Prior to restart of the plant, we

concluded that the plant could be safely operated.

Since our focus was on safety i$sues impacting on r~starting the plant,

enforcement action was generally not considered in this inspection.

Any

enforcement action that may result will be pursued subsequent to receipt of

your response to the DET report.

In the course of this inspection, we also

observed a number of significant weaknessesi as identified by the DET, which

required management attention. The weaknesses observed included additional

examples of inadequate correct~ve actions, degraded and potentially inoperable

safety systems, long standing deficiencies, and missed commitments.

These

will require long term corrective actions and will also be reviewed in future

inspections which will address DET issues including your respon~es.

In accordance. with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter and the enclosures will be placed in the NRC Public Document Room.
