During the period from November 1, 1993 through April 30, 1994, one NPDES State notification was required and one report of Oil, Salt or Polluting Material Loss was made.

The Attached Non-Radiological NPDES permit violation report contains a summary of the recent NPDES Permit activity and copies of the notifications sent to the State of Michigan.

NON RADIOLOGICAL NPDES PERMIT VIOLATION REPORT NOVEMBER 1, 1993 TO APRIL 30, 1994 NPDES NOTIFICATIONS During this period between November 1, 1993 to April 30, 1994, there was one NPDES notification reported to the Michigan Department of Natural Resources

{MDNR). The reported incident is described as follows:

On Sunday, April 10, 1994, the plant exceeded the Total Residual Oxidant

{TRO) limit of 0.3 ppm TRO during chlorination of the service water bay.

The plant obtained an outfall sample which was analyzed to have 0.47 ppm TRO. Chlorination was immediately secured. An observation of the outfall was made to determine the extent of fish kills or fish in distress, none of which were observed. Exceeding the TRO limit was a result of the diversion of chlorine to the mixing basin via the dilution water pumps.

This situation occurs only when the plant is off line and was not previously considered. The diversion of chemical to the mixing basin near the plant's outfall greatly decreased the amount of contact time for the chlorine within the plant. A copy of the NPDES notification report to MDNR is attached.

Oil, Salt, or Polluting Material Loss During this period there was one report of Oil, Salt, or Polluting Material loss. There was a loss of approximately 60 gallons of sewage to a nearby storm drai~ on March 24, 1994. A copy of the notification to MDNR is attached.

Other Activities Other activities which may be of interest pertaining to water quality are summarized as follows:

On March 25, 1994, an NPDES permit renewal application was submitted to MDNR. The permit renewal application was submitted for renewal of Palisades NPDES Permit No. MI0049131. This NPDES permit is due to expire on October 1, 1994. In order to have no interruptions in the service of Palisades Groundwater Remediation System, the NPDES permit renewal application needed to be submitted to MDNR by April 1, 1994.

1. Date of Incident: April 10, 1994
2. Identification of Specific Effluent Limitation or Monitoring Requirement Affected:

Daily maximum limit of 0.3 ug/l total residual oxidant (IRO).

3. Description of Incident:

During intermittent chlorination of the plant service water bay on April 10, 1994, a TRO reading of 0.47 ug/l was measured. During this chlorination, duplicate analyses of TRO were being performed on different titrators as a quality control check. A problem was noticed in the identification of the endpoint of the TRO titration for one of the instruments.

4. Apparent Cause:

The problem with identification of the*TRO titration end point resulted in the NPDES TRO limit for chlorine apparently being exceeded for less than 15 minutes.

5. If not immediately corrected, the anticipated time the condition is expected to continue:

The intermittent chlorination of the plant's service water bay was immediately terminated following the analysis which indicated the elevated TRO level.

6. Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Evaluation of TRO analytical and procedural steps will be concluded by 4/18/94 and will include evaluation of the analytical procedural steps, identification of TRO titration endpoint and evaluation of.

locations for chlorine addition to the service water bay.

7. Additional Comments:

Visual observations of the Plant's mixing basin and Outfall 001 discharge to the Lake at the time of the chlorination inqicated no adverse impact on fish or other aquatic biota.
