SUBJECT:
NRC INSPECTION REPORT NO. 50-255/94006
This refers to the routine safety inspection conducted by Messrs. M. E.
Parker, D. G. Passehl, K. Salehi, J. F. Schapker, and C. N. Orsini of this
office from March 15 through May 9, 1994.
The inspection included a review of
activities at your Palisades Nuclear Generating Facility. At the conclusion
of the inspection, the findings were discussed with those members of your
staff identified in the enclosed report .
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The purpose of the inspection was to determine
whether activities authorized by the license were conducted safely and in
accordance with NRC requirements.
No violations of NRC requirements were identified during the course of this
inspection. However, some concerns were identified when a spent fuel assembly
became stuck in its spent fuel pool rack location during withdrawal for
ultrasonic testing on March 11, 1994. Although the spent fuel assembly was
successfully recovered without incident, there were inadequate procedures to
ensure continuous manned coverage and monitoring of spent fuel pool water
level.
We noted that you had implemented appropriate procedural changes to
prevent similar occurrences in the future, and will continue to observe spent
fuel pool activities during future inspections.
