This refers to the inspection conducted* by Mr. J. F. Smith and others of this
office on Se.ptember 20 through November 27, 1993.
The inspection included a
review of activities for your Palisades Nucle~r Power Plant:
At the
conclusion of the inspection, the findings were discussed with thos~ members
of your staff identified in the enclosed report.
The areas examined during the inspection are identified in the report. ' These
areas consisted primarily o.f activities related to the cracks found inthe
pressurizer power operated relief yalve piping.
Within this area, the
inspect-ion consisted of a selective examination of procedures and
reptesentative records, observations, and intervie~s with personnel.
No violations of NRC requirements were identified during the course of this
.inspection.
An appar~nt lack of management involvement was noted early in the repair
effort as evidenced by a slow response in providing a comprehensi~e
inspection/repair plan. Also, your consideration of the generic aspect of
- this PWSCC failure was considered weak~-
In accordance with-10 CFR 2.790 of'the Commission's tegulations, a copy df
this letter and the enclosed irispection report will be placed in the NRC
Public Document Room.
