Rill-93-010, by this letter I hereby notify the NRC Region III Regional Administrator that Consumers Power Company has completed the actions addressed in Confirmatory Action letter Rill-93-010.

Consumers Power Company has completed those actions by:

I.a. Conducting an investigation to determine the r.oot causes of the damage to fuel bundle 1-024 and the inadvertent lifting of fuel bundle SAN-08 with the upper guide structure (accounting for the similar events in 1988 and 1992}.

b. Developing appropriate corrective actions to prevent recurrence of these types of events.
c. Maintaining evidence of the investigations and corrective actions referred to in 1.a and 1.b.
2. Having an independent safety assessment performed to review the facts surrounding the damage to fuel bundle 1-024 and the recent and previous inadvertent lifting of fuel bundles with the upper guide structure. The assessment also determined that our root cause analyses and corrective actions are appropriate.

'i 3.a. Conducting a prompt assessment of problems experienced during refueling and fuel handling activities to determine what near-term corrective actions were necessary prior to resumption of these activities. The prompt assessment considered:

1. Adequacy of management oversight and operator training; and
2. Adequacy and adherence to procedures.
b. Reviewing the results of this assessment and near-term corrective actions with the NRC's Augmented Inspection Team.
4. Maintaining, after receipt of the.CAL, senior site or corporate managers onsite at all times to visually observe work activities in progress throughout the refueling outage. This action was discontinued at the time the plant left cold shutdown after refueling. However, senior site managers have been assigned to observe important evolutions.and testing during startup and power ascension. Additionally, a continuing program of senior management field observations has been instituted to monitor routine activities during operation.
5. Being prepared to quarantine equipment determined to be appropriate by the Augmented Inspection Team (AIT). (No equipment was requested to be quarantined by t~e AIT.)
6. Meeting with Senior NRC Management on September _9, 1993 to discuss the results of the above referred to assessments and*correcti.ve actions.

Documentary evidence of the above actions is available to the NRC.
