Ml 49043 September 2, 1992 Nuclear Regulatory Commission Document Control Desk Washington, DC 20555 DOCKET 50-255 - LICENSE DPR PALISADES PLANT - OPERATOR AND SENIOR OPERATOR LICENSING EXAMINATIONS - SUBMITTAL OF NRC FORM 398 By letter dated June 3, 1992 entitled "Operator and Senior Operator Licensing Examinations" the NRC requested that Palisades submit certain documentation for the administration of licensing examinations at the Palisades Nuclear Plant.
By cover letter dated July 7, 1992 Palisades submitted the required reference material to the Region III staff.
The NRC's June 3, 1992 letter also requested that the licensee submit preliminary reactor operator and senior reactor operator license applications and medical certifications 30 days prior to the examinations dates.
By letter dated August 4, 1992 NRC Form 396, "Certification of Medical Examination by Facility Licensee," and preliminary, unsigned copies of NRC Form 398, "Personal Qualification Statement - Licensee" were forwarded to the NRC Regional Administrator - Region III.
